$150 Week/Vouchers Accepted   *Trips Are Additional Fee

Submit Required Documents 
All required documents are to be emailed to lamcampuskids@gmail.com. No documents will be accepted on site.
*Please Note you do not have to resubmit Birth Certificate and Insurance Cards if already on file

4. Procare Payment Form (will be emailed to you after your registration is approved. *All parents both private pay and voucher are required to complete this form accurately. Failure to complete this form will end your child(ren's) enrollment with LAM.
